We welcome you to Therapeutic Focus and look forward to working with you and your child or children! To help prepare for your first visit, please take a few minutes to review the following information.
Please arrive 10-15 minutes early to your first appointment to allow time to complete new patient intake forms if you choose not to complete it electronically.
If you have any of the following, please bring them to your first appointment.
Your insurance card(s)
A government-issued photo ID
Referral/Rx from your doctor
Written reports from physicians
List of all medications currently taking
Results from any diagnostic studies, such as X-rays or MRI services that pertain to your condition
